You may visit a health care organization in person or virtually in order to make your final choice about the health information technology system or health information technology application of interest. Choose the best strategy to gain information about your selected information technology system/application. Some ways to gather information include virtual visits; vendor demonstrations; on-site visits; interviews via face-to-face, phone, or teleconference. You must conduct at least one interview for this project. Complete a literature search to gather information about your selected information technology system. You may also need to review related scholarly articles to help answer the questions presented below. Organization Information · Briefly describe the health information technology system/application and the organization type (hospital, clinic, public health agency, health care software company, government health information website, private virtual health information site, etc.).  Within our hospital, our EHR is Meditech and we also use centricity PACS for imaging which interfaces with the system. The EHR covers nursing and provider aspects by providing clinical documentation and ordering solutions to improve patient care and the clinician’s experience. Data can be manually entered and processed within the system and also data is entered as it interfaces from other systems. Numerous reports can be generated for example sepsis reports etc. decision support is used with medication ordering as well as medication administration where clinicians receive alerts if there are conflicts with medications such as duplicates or allergy warnings. Is the health information technology system/application clinical, administrative, educational, or research related? Clinical What were the key reasons for the development of this health information technology system/application, i.e., what made the organization believe this system/application was needed? How did this organization determine those needs? Did the organization use specific tools to conduct needs assessments, staff opinions, or workflows? The electronic Health Record system was developed to help deviate from paper charts which have become less secure over the past few years, and also to maintain compliance with CMS guidelines. Meditech covers CPOE, nursing and providers notes, eprescribing, medications administration as well as billing. PACS interfaces with Meditech to allow images to be available to providers. The system is very efficient and provides users with a user friendly interface. It integrates very well with other applications and allows the delivery of information to be relatively smooth and accurate. There are a number of features available that help end users with their workflow and hence helping to increase patient outcomes. How did the organization determine that this specific system/application could fulfill its predetermined needs? Based on workflow assessments of all clinical entities and stakeholders that would be directly impacted by the implementation of this EHR. Who manages this health information technology system/application and where are they located within the organization’s administrative structure? The organizational structure is composed of the corporate level and division level. There are individuals who manage different aspects of the system, for example, the Clinical team handles only the clinical aspects, such as documentation, screens, ordering process etc. the technical team handles all technical Information System Application Design and Development Many health care systems have multiple independent entities that work together toward the common goal of providing high-quality care. How did—and do—the various stakeholders make decisions related to this health information technology system/application? Were the end users involved in the development of this health information technology system/application?  The general project may be generated on the corporate level and trickle down to the division level which then gets dispersed to each facility within the division. Different aspects of the project may require specific stakeholder input and feedback especially if the project will impact providers (CMO/Lead Physician), or nursing (CNO) etc. How are individuals trained to use the health information technology system/application? Either via live 1:1 training or virtual classes How are security issues addressed?  Appropriate access audits as well as other random security audits are conducted frequently. How does this health information technology system/application support a legally sound health care record? Where did initial funds for this health information technology system/application come from? We got reimbursed from CMS post CPOE rollout. But initial would have been from the corp level to get the project going. Who manages the budget for this health information technology system/application? All budget is managed on the Corporate Level Have organizational or political issues impacted the ongoing funding for this health information technology system/application? NA What are the arrangements for planned or unplanned downtime? Communication is sent to all facilities that will be affected by the downtime if planned along with instructions on what procedures to follow. For unplanned downtimes, facility already aware of downtime procedures to implement. How are health information technology system/application upgrades scheduled or planned? Communication/education is sent to all units that will be affected at least 2 weeks -3 weeks in advance. Any downtime associated with the upgrades are also communicated. How has the health information technology system/application changed in response to health care reform and related legislation? All regulatory initiatives and mandates are handled at the corporate level and decisions are made based on these mandates that impact the changes needed to the system. Documentation updates as well as ordering mechanisms have changed over the years according to these governmental mandates/requirements. What suggestions could you make regarding changes needed to support health care reform and related legislation? Innovative Aspects of the System How does the health information technology system/application utilize technology innovations? As technology advances, the organization finds ways to incorporate these advancements into our current workflow. We needed an easier more efficient way to communicate amongst each other as well as among departments and providers. Hence the implementation of a communication application that was rolled out to all clinical and non-clinical persons. As needs arise, new technological innovations are implemented.
