At age 72, he had a long history of chronic obstructive pulmonary disease, coronary artery disease, three myocardial infarctions, liver disease secondary to alcoholism, and an amputated right arm from an automobile accident. He was admitted with renal failure and placed on a medical surgical unit for the weekend. He was disoriented and lethargic but was cooperative with most procedures and hospital routines. Bethany H, one of the RNs on the unit, was assigned to supervise the care for Mr. T during her 12-hour shift. Early in the morning, Mr. T’s most recent ex-wife came in to visit and help with his morning care. Even though they had been divorced for 10 years, they had remained close and spent a great deal of time together. This former wife informed Bethany that Mr. T had told her many times that if anything should happen to him, he did not want to be kept alive with tubes and machines. Later in the day, Mr. T’s son and daughter by an earlier marriage arrived from out of state. They had not seen their father in several years and were appalled at how much he had deteriorated. They requested to speak to the physician about Mr. T’s condition and prognosis, but his attending physician was off call for the weekend. The physician taking the call did not feel familiar enough with the case to discuss it with them, either on the phone or in person. The son and daughter told Bethany that they wanted everything possible done for their father until they got a chance to talk to the attending physician. Mr. T’s condition deteriorated rapidly during the shift, and by midafternoon he was totally unresponsive, his blood pressure was 72/40 mm Hg, his pulse rate was 144 bpm, and he had not had any urine output for 5 hours. Bethany called the on-call physician to see about a do not resuscitate (DNR) order based on Mr. T’s former wife’s account of what the patient had requested. The physician said that although a DNR order seemed appropriate for this terminal client, he was not sure he could give it because he did not know the client that well. He wanted to think about it some more. The physician did order some furosemide (Lasix), an increase in the IV rate, and some blood work. Just before the end of Bethany’s shift, Mr. T’s former wife called to see how he was doing. When informed of his critical condition, she begged the nurse to let Mr. T go peacefully.
