1. The scenario involves a situation where a patient has been hospitalized and as part of his discharge instructions, he is told to schedule a follow-up lab test (cardiac panel) and to make an appointment with the cardiologist to be seen within 3 business days.

On discharge, the patient calls the cardiologist’s office and is told that the doctor is out of town until next week, so the appointment is made for then. As the discharge instructions also called for laboratory work to be performed, the patient shows up at the local lab at 7 am for fasting blood work only to be told that there are no orders in the computer for him/her.
For this exercise, complete the following:
• Using a 3 column format, list any quality issues, identify the affected stakeholder, and why do you perceive that these may be quality issues
• Process map the current process for each of these scenarios
2. Using the scenario above and your process map, create a fishbone diagram.
3. Using the scenario above, determine how you will gather data, over what period of time, and what measures will you use to determine the value of your data, and what type of chart will you suggest.
4. A run chart will be provided and with that will be a list of questions to be answered. The questions require short responses.
5. You are the QA manager and have reviewed several scenarios regarding the transition between hospitalization and outpatient services. Based on your knowledge, you will provide leadership with a Quality Plan and chartering.
