Forms & Queries: Create a minimum of 1 user-friendly data entry form for Patients, Doctors, Appointments, and/or Prescriptions. Develop a minimum of 5 Queries to retrieve specific information from the database. These queries should answer common questions, such as finding patient appointments on a specific date, a doctors schedule of patients for the day, etc..
Reports: Design a minimum of 2 reports to present information in a formatted and printable manner, such as patient records or appointment schedules.
Data Population: Populate the database with sample data to demonstrate its functionality. Minimum of 10 fictional records in your patient and/or employees table(s). And a minimum of 1 record in all other related tables (primary key counts).
Once your database is complete with the requirments from above, prepare to submit it here for your Final Project grade. Your file must be a Microsoft Access database file (.accdb). The database should be functional, containing sample data and demonstrating the ability to perform key tasks. The project will be assessed based on the quality of the ERD design, the functionality and structure of the database, the accuracy of data retrieval using queries, and the completeness of user documentation.
