The assignment has 3 parts; please allocate up to 5 pages in total to complete all three parts. Service Plan The Service plan will outline how you plan to meet the goals identified in Part A. This will often include facilitating relevant referrals to supports in the community. The supports can be formal or informal, or a combination of both. The plan can also include actions such as form filing, advocating with existing or new service providers and coordinating care. For each goal you should have 2-3 3 objectives (Examples can include referrals, providing resources or action items attached that demonstrate how the goal will be carried out). *information for one relevant referral however must be included per goal. One form indicating your goal for housing with two to three objectives explaining how that goal will be met. Advocacy Letter Referrals to services are often challenged by long-wait lists or by failure to meet the eligibility criteria. This can cause problems for a client who may be in crisis or in need of expedited connection to resources. This section will ask you to complete an advocacy letter requesting the need for service. You will be assessed on your ability to articulate a convincing case as to why the client’s referral should be prioritized. Alternatively, you may write a letter challenging a denial of service to the client. Your letter should be concise with a clear argument for your client. This letter should be ideally be no longer then one page single spaced. More then two pages should not be submitted.
