What need(s) do you identify as requiring to be addressed by the community in collaboration with the community health nurse and other community partners?

I am doing a windshield survey on East Harlem community. I would like to answer the question in paragraph format including the following points.1. Education: read labels, food choices (choosing healthy food will reduce the obesity rate and its comorbidities thus improving the quality of life of the community members) 2. Exercise programs: in school and community center 3. Safety: neighborhood watch group to prevent crime 4. Prevention: increase health literacy and awareness 5. Early intervention: connection to resources (advocating patient to other resources, give some examples), health screening/ fairs (blood pressure, blood glucose, hearing, vision tests etc.)
