Identified Problem; Healthy People 2020/ PH Intervention Wheel 10 points Briefly state the problem you identified and the most relevant Healthy People 2020 measurable objective related to your project. Describe what you did using the interventions from the Public Health Intervention Wheel. 5. Evidence-Based Practices: 10 points Provide a concise summary of the scientific evidence you used to justify your intervention. (See previous SLP papers) 6. Planned Intervention & Theoretical Framework/Conceptual Model 10 points Provide a summary of the intervention and the a theoretical model or conceptual framework that guided your project. 7. Evaluation Process (10 points) Provide a sample of the evaluation format you used (Pre/Post Test, interviews, change in attitude) and the results. 8. Final Reflections 15 points Think back to your personal learning objectives when you began your community assessment. How did the Service Learning Project experience address your learning objectives?
