Part one (ROI form Review)
Review the release of information forms attached to this assignment. If you have one from your place of employment, feel free to substitute that for one of those attached. If you select a for from your place of employment, please be sure to submit it along with your assignment. Review all of the ROI forms for completeness and compliance with HIPAA. In your review, be sure to specifically address the forms’ compliance with HIPAA as it relates to psychotherapy notes, mental health and HIV. Submit your critique.

Part two (Training Module)
Create a training module for a new Release of Information employee on the completeness and compliance with HIPAA as it relates to a release of information form. The module should address the completeness of an ROI form and compliance with HIPAA. Please also include information on compliance as it relates to substance abuse, mental health, and HIV. You should assume that your facility provides mental health services and substance abuse treatment. This training should be at least 12 PowerPoint slides long.

Part Three (ROI policy)
Search online for samples of release of information policies. Create a release of information policy that specifically includes the training module that you have created in #2.
