Chronic Disease Infographic
Chronic diseases are a persistent issue in the U.S. Sixty percent of the population lives with at least one chronic disease, and many live with more than one. No community, city, or state is free from chronic disease; your community is not an exception. In this Competency Assessment, you will research the chronic diseases that are most prevalent in your community using data and statistics, locate the programs that are currently available to help address one of the chronic diseases, and create an infographic to help educate a population within your community on prevention, detection, and treatment for the issue.
Step 1
Research the chronic diseases that are most prevalent in your community. You can find this information in various ways including: Please use diabetes or heart Disease
Community: Kern County in California or the State of California
• Online data and statistics
• Contact the local health department
• Ask members of the community
Step 2
Select one chronic disease to address
• Identify the community/population that is most affected by the selected chronic disease. Please use either diabetes or heart disease.
Step 3
Research programs that already exist to help address the selected chronic disease (e.g. The Alzheimer’s Association, education programs, early-stage engagement programs).
Step 4
Develop an infographic to help educate your selected population on prevention, detection, and treatment for the chronic disease
• Include at least one piece of supporting statistics or data
