Conclude with how your proposed project will help to expand the previous literature or a prior program to reach more people.Please Note: The background of your selected issue and significance of your project can be between 3 to 4 paragraphs in length.4) Innovation: Describe how your provision of resources or program will enhance either healthcare, education, and/or research.- If you are providing resources, how will this improve patient care outcomes? Will this reduce specific risk factors for any chronic health conditions? Will this improve healthcare services for the community? How could the resources you provide be sustainable across time if you receive funding (e.g. what indicators could you trend to help you determine if your community is being reached through the resources you are providing?)- If you are implementing a health education program, will this improve education on specific risk factors and health behaviors that place a person at risk for one or more health issues? Does the program communicate health information in an easy-to-follow format across the spectrum of health literacy? Does your program increase awareness for your public health issue of interest? What kind of team members will be involved in the delivery of your health education program?Please Note: The Innovation section can be between 3 to 4 paragraphs in length.5) Project Design and Evaluation: Provide a description of how you will complete a community health assessment and recruit participation from your selected community.
