You will need to access the Joint Commission Manual for Hospitals in order to complete this assignment. You are the HIM Director at Acme Hospital a 300 bed facility. Acme specializes in cardiac care and treats approximately 750 patients a month. You have been tasked with designing a chart audit tool to show compliance with these standards. Select 10 standards that you feel are important to illustrate compliance with documentation within the medical record. This chart audit tool (form) should act as a data collection tool for analysis of compliance. Some things to think about while you are designing your form: What patient identifiers will you include? While meeting ALL of the standards is important, what 10 do you think are the most important? 1. The first part of the assignment is to review the Joint Commission manual for standards that pertain to medical record documentation. These should be specific standards, items that deal with topics about signing documents, completeness of documents, medical record specifics, etc. 2. Then you want to develop an audit tool with those 10 standards you have selected to assess records in the organization. The chart audit tool should be designed in a way that it will best help the organization in data collection to determine if the organization is meeting or not meeting the standards. This tool can be developed in any application.3. The third part of the assignment is a brief summary of your recommended process. How often the audit tool should be used, how many records to be reviewed, how often and by who?, Who will perform this audit? How many charts will be audited? How often will the audit be performed? Will the form need to be adapted for specific units? Such as pediatrics and newborns? If so, what would you suggest for these units? If not, why not?
